Could be a bug. Might also be an effect of turning off aging, if you've done that for your sims. How many days I must wait for the automatic growth?
Cause my baby after 8 days is still in the cradle I really don't know what to say on the hospital thing. I'll have to take a screenshot. And no, there's no physical hospital. As far as I can tell there's absolutely no difference whatsoever between having the baby at home and having it in a hospital. Don't honestly know if that's the case here. I've held off on replying to this mainly because I'm not sure if that's the only prerequisite.
I'll have to tinker before I can give a definitive answer. Normally, though, yeah, it's just clicking and choosing. Just click on the pregnant Sim that's going into labor and you'll get an option to give birth in a hospital.
